What you'll need to apply

• Income tax returns OR recent pay stubs (documentation limited to these for Charity/Discounted Care eligibility)
• Bank/checking account statements evidencing available resources (base FAP)
• At least one piece of supporting income documentation (base FAP: tax return, W-2, paystubs, or signed letter of support)
• Documentation of Essential Living Expenses (only if patient requests an extended payment plan)
• Asset documentation may be accepted but shall NOT be required

When you call, say: “I'd like to apply for financial assistance (charity care) for a hospital bill. Can you send me the application and tell me which documents I need?”
