What you'll need to apply

• Completed Financial Assistance application
• Recent pay stubs (within 6-month period before/after first billing, or when preservice application submitted)
• Copy of recent Federal Income Tax Return (Form 1040) for members of the Patient Family
• If paid in cash: letter from employer providing rate of pay
• If no employer/employee arrangement: other written documentation of in-kind income (case-by-case)

When you call, say: “I'd like to apply for financial assistance (charity care) for a hospital bill. Can you send me the application and tell me which documents I need?”
