• At least one piece of supporting documentation verifying Family Income (base FAP)
• For CA: documentation limited to income tax returns or recent pay stubs
• Documentation of assets may be accepted but shall not be required (CA)
• Documentation of Essential Living Expenses if requesting an extended payment plan (CA)
