What you'll need to apply
- Income tax returns OR recent pay stubs (for Charity Care / Discounted Care eligibility, documentation limited to these)
- Bank or checking account statements evidencing available resources (per base FAP)
- Current Form W-2 (base FAP)
- Signed letter of support (base FAP)
- Documentation of Essential Living Expenses if requesting an extended payment plan
- Assets documentation may be accepted but shall NOT be required
