What you'll need to apply
- Information about family and family size
- Family's gross monthly income (before taxes and deductions)
- Documentation for family income (most recent tax return incl. schedules; OR W-2; OR current pay stubs (6 months); OR written signed statements from employers/others)
- Approval/denial of eligibility for Medicaid and/or state-funded medical assistance OR approval/denial for unemployment compensation
- Documentation for family assets (current bank statements showing most recent 3 months for checking/savings; investments incl. stocks and bonds; trust funds; money market accounts; mutual funds; other penalty-free withdrawable investment funds)
- Signed and dated form
