How to negotiate a hospital bill in California
A hospital bill is rarely the final word. In California you have the right to charity care, discounts, and a fair payment plan — and a few specific steps can lower what you actually owe. Here's how to negotiate, in order.
Step 1: Apply for financial assistance first
Before you negotiate a price, find out whether you owe anything at all. Under California's Hospital Fair Pricing Act, hospitals must give free or discounted care to eligible patients, and eligible patients cannot be charged more than the amount Medicare or Medi-Cal would pay. Step 2: Get an itemized bill and check it for errors
Ask the billing office for a fully itemized bill that lists every charge. Billing errors are common — duplicate charges, services you never received, or the wrong billing code. Compare it to what you remember and question anything that looks wrong. Fixing an error can cut the bill before you negotiate anything else.
Step 3: Ask for the uninsured or cash discount
If you're uninsured or paying yourself, ask directly: “What is your discount for uninsured or self-pay patients?” Hospitals often have a standard uninsured discount, and the first price you're quoted is rarely the lowest. Be polite, be specific, and ask them to put any agreed discount in writing before you pay.
Step 4: Set up a payment plan you can actually afford
If you still owe a balance, ask for an interest-free monthly payment plan based on what you can realistically pay. Under California law, payment plans for eligible patients must be reasonable. Don't put the bill on a credit card or a medical credit card — that can turn protected medical debt into regular debt with interest. Get the plan in writing.
